چکیده

Impaired exercise capacity is the core symptom of heart failure with preserved ejection fraction (HFpEF). We assessed effects of exercise training and Crataegus extract WS 1442 in HFpEF and aimed to identify mechanisms of action in an exploratory trial (German Clinical Trials Register DRKS00000259). 140 sedentary HFpEF NYHA II patients on standard treatment received eight weeks of aerobic endurance training and half were randomized to WS 1442 900 mg/day. Symptoms, 2 km walking time (T2km), parameters of exercise tolerance, cardiac and vascular function, muscular efficiency and skeletal muscular haemoglobin oxygen saturation (SO2) measured during a treadmill protocol were captured at baseline and after eight weeks. Adverse events were recorded during the trial. Mechanisms of action were explored by correlation and path analyses of changes. Symptoms and exercise capacity improved with training, but correlations between OPEN ACCESS

The efficacy and safety of Crataegus extract WS 1442 in patients with heart failure: the SPICE trial.

BACKGROUND Crataegus preparations have been used for centuries especially in Europe. To date, no proper data on their efficacy and safety as an add-on-treatment are available. Therefore a large morbidity/mortality trial was performed. AIM To investigate the efficacy and safety of an add-on treatment with Crataegus extract WS 1442 in patients with congestive heart failure. METHODS In this ra...

Benefit-Risk Assessment of Crataegus Extract WS 1442: An Evidence-Based Review

Preparations from Crataegus (hawthorn) have a long history in the treatment of heart failure. WS 1442 is a dry extract from hawthorn leaves with flowers (4-6.6:1), extraction solvent of ethanol 45% (w/w), adjusted to 17.3-20.1% of oligomeric procyanidins. Nonclinical studies show that WS 1442 has positive inotropic and antiarrhythmic properties and protects the myocardium from ischemic damage, ...

Effect of Exercise Training on Heart Rate Variability in Patients with Heart Failure After Percutaneous Coronary Intervention

Background: This study aims to evaluate the effect of exercise training on heart rate variability (HRV) and to determine the correlation between parameters of HRV and the ejection fraction in patients with heart failure after percutaneous coronary intervention.Material and Methods: Fifty patients with left ventricular ejection fraction ≤ 40% undergone percutaneous coronary intervention were r...

Changes in the structure of the heart and blood pressure after a period of concurrent endurance-resistance training in patients with chronic heart failure

Background: Exercise improves functional capacity by improving endothelial function and musculoskeletal aerobic metabolism, but its effects on hemodynamic control are not well known. The aim of this study was to investigate the effect of aerobic and resistance trainings on structural changes and blood pressure in patients with heart failure. Materials and methods: 76 patients aged 65 to 79 year...

Exercise training in patients with heart failure and preserved ejection fraction: meta-analysis of randomized control trials.

BACKGROUND Heart failure with preserved ejection fraction (HFPEF) is common and characterized by exercise intolerance and lack of proven effective therapies. Exercise training has been shown to be effective in improving cardiorespiratory fitness (CRF) in patients with systolic heart failure. In this meta-analysis, we aim to evaluate the effects of exercise training on CRF, quality of life, and ...
